Accountable Care Collaborations
Valley Preferred is actively involved in accountable care initiatives with major payer partners. These partnerships focus on creating a more coordinated patient care experience and improving health outcomes, which can positively impact costs for members who see Valley Preferred accountable care network physicians.
Aetna and Valley Preferred are engaged in an accountable care agreement designed to improve the quality of care and patient experience for members of fully insured Aetna Whole Health℠ commercial plans in Lehigh and Northampton counties. Aetna offers a wide array of co-branded Aetna Whole Health plans, giving employers affordable choices that enhance the membership experience through a collaborative team approach to the full spectrum of health care. Aetna Whole Health members receive the highest level of benefits when accessing care through Valley Preferred accountable care network doctors. Providers assume responsibility for improving the quality of patient care and are paid, in part, to keep their patients healthy. The organizations collaborate to better coordinate care, and quality efficiency measures are jointly pursued.
The accountable care agreement with Cigna is designed to improve patient access to health care, enhance care coordination, and achieve the goal of improved health, affordability and patient experience. The program is Cigna’s first collaborative accountable care initiative to launch in Pennsylvania. Registered nurses are critical to the program’s success, as they serve as clinical care coordinators and help patients with chronic conditions or other health challenges navigate the health care system. The nurses are aligned with a team of Cigna case managers to ensure a high degree of collaboration. Accountable care network physicians may be rewarded through a pay-for-value structure for meeting targets for improving quality and costs through efficiencies.
