Participation - ACE Program
In addition to delivering exceptional care in their daily work, LVPHO member physicians are eligible to receive incentives for participation in designated Achieving Clinical Excellence® (ACE) program activities.
Quality Improvement Project
Our Quality Improvement Project provides member physicians with the opportunity to achieve new levels of health care delivery, efficiency and accuracy, and to pursue these improvements by conducting preliminary research and recommendations on procedures to achieve new best-practice standards. Physicians may submit QI projects demonstrating improvements in quality and efficiency of patient care. Written criteria and requirements for QI projects can be furnished upon request. Since the program’s inception in 2009, multiple QI projects have been initiated or completed.

Completion of Provider Self-Audit (PSA)
Audits are sent to member physicians for analysis of quality care (patients they care for). Incentive is awarded for payable audits. Audits are prioritized by disease to include: CHF, COPD, Diabetes, Asthma, Pediatric Asthma, CAD, Hypertension, General Child and Adolescent Preventive, General Preventive Health.
Independent Medical Review
Populytics Medical Director may request clinical consultations on medical management from member physicians. This can be done as a written or telephonic consultation.
Member physicians are encouraged to attend Greater Lehigh Valley Independent Practice Association (GLVIPA) membership meetings.
Member physicians can meet with the LVPHO Medical Director, an Associate Medical Director and/or a Physician Advocacy Liaison to discuss initiatives, outcomes and incentives.
Patient Centered Medical Home NCQA Recognition
Member physicians listed in practices, that become a recognized Patient Centered Medical Home with NCQA level distinction, are incentivized. Note: Physicians must be listed on the NCQA application.
Invited Speaker to Care Management Committee
Member physicians are invited to present their conclusions of a finished Quality Improvement project.
Clinical Integration Education Program (CIEP)
Member primary care and specialty physicians interact and stay connected through education presentations enhancing the way care is integrated. Attendance is incentivized once per quarter, however speaking opportunities are unlimited. The provider integration must be requested and approved prior to the Clinical Integration Education Program (CIEP) session. CIEP sessions are defined by the request and categorized into forum-based sessions or practice-based sessions.
Institute for Healthcare Improvement Quality Course
Member physicians are encouraged to take online courses dedicated to Quality Improvement, Patient Safety, Triple AIM, Cost Containment and Leadership. Incentive is for completion of the 6 Quality Improvement modules (QI 101- QI 106). In addition, LVHN will contribute toward the IHI membership fee and LVPHO members will receive CE credits toward the 12 patient safety credits that must be obtained every two years for board certification. Course registration instructions can be furnished upon request.
Choosing Wisely® TLC Education Module
Member physicians can earn CME credits for completion of online training modules pertaining to the Choosing Wisely campaign. Course registration instructions can be furnished upon request.
