Preferred Health Management, available exclusively through the Lehigh Valley Physician Hospital Organization (parent organization of Valley Preferred), is a comprehensive health care management system proven to contain health plan costs while delivering the highest quality of care on a consistent basis. Preferred Health Management provides on-site health and wellness programs, disease management, case management and other health-related services.

Physician engagement.

Disease specific audit tools were developed using evidence-based medical research to help practitioners track their own performance. PHM spans the continuum of patient care from prevention through the ongoing maintenance of at-risk patients:

Utilization Review: Preferred Health Management utilizes medical guidelines to determine the appropriateness of medical services, specifically monitoring inpatient levels of care, drug utilization, rehabilitation therapies, use of durable medical equipment and other services to provide high quality care in a cost-effective manner.

Case Management: A nurse will develop a health care strategy that meets the specific needs of the individuals suffering from a catastrophic illness or injury while coordinating and monitoring all aspects of care to achieve the goals of an optimum health care outcome.

Disease Management: Preferred Health Management takes the most costly chronic conditions and manages care through patient specific assessment, coaching and education to minimize complications, while promoting quality of life, avoiding exacerbation of disease and associated costs. More than 70% of the health care costs in our service area are consumed by individuals with diabetes, asthma, hypertension and heart disease. Preferred Health Management targets these and other conditions with programs clinically proven to reduce the effects of major chronic diseases.

Wellness Programs: Employees are provided with a variety of educational offerings, including a Health Risk Assessment awareness tool to make the progression toward better health a way of life.
