your understanding of your current health status
and to assist you in identifying health risk
factors, Valley Preferred is offering you a variety
of health assessment surveys that will guide
you through a simple series of questions in various
categories. These surveys include: General Health
Assessment, Health Risk Assessment (HRA), Cardiac
Risk Assessment, Diabetes Risk Assessment, Fitness
Assessment and Well-Being Assessment.

by your employer) is needed to participate in this
