party administrators seeking access to our network for their
clients or products. When choosing Valley Preferred as a health
care partner, we require:

cards |

A network management fee |

Preferred Health
Management (PHM) services, available exclusively
through the Lehigh Valley Physician Hospital Organization
(parent organization of Valley Preferred), is a comprehensive
health care management system proven to contain health
plan costs while delivering the highest quality of care
on a consistent basis. |

The benefit plan design must have some financial incentive
for Valley Preferred on an “in network” level |
| • |
All claims rendered in our network to be repriced by
Valley Preferred |
| • |
Accurate up-to-date eligibility data on all members accessing
our network |
