For Members:
It’s easy and convenient! Answer a series of simple questions to improve your understanding of your current health and help identify health risk factors. To begin, please click here. A registration/location code (provided by your employer) is needed to participate in this online survey. If your employer is not participating with Valley Preferred, please try our free Health Calculators.

Members can save and print a report of their results to share with their physician. Once you have taken the Health Risk Assessment, you will have access to a General Health Assessment, Cardiac Risk Assessment, Diabetes Risk Assessment, Fitness Assessment and Well-Being Assessment.

For Employers:
Valley Preferred makes it easy to help your employees assess their health risks. With our Health Risk Assessment, they can answer a series of simple questions to improve their understanding of their current health and help identify health risk factors. Once the Health Risk Assessment is completed, your employees will have access to the General Health Assessment, Cardiac Risk Assessment, Diabetes Risk Assessment, Fitness Assessment and Well-Being Assessment.
